·  NAMB Mortgage Broker Contract

·  Mortgage Broker Fee Disclosure

·  Lock-in Agreement

·  Waiver of Escrow

·  ECOA, Reg. B

·  Fair Lending Notice

·  Private Mortgage Insurance Disc. (Pages 1 & 2, Generic)(if applicable)

·  FL- Mortgage Broker Contract (Pages 1-3)

·  FL- Equal Credit (Pages 1 & 2)

·  1003 Application (Pages 1-4)

·  Request for Tax Form 4506 (Pages 1 & 2)

Collect the following documents:

·  Pay Stubs for the last 2 pay periods

·  Bank statements for previous 2 months

·  Tax Returns or W-2s for the past 2 years

Refi documents

·  Copy of Title

·  Copy Warranty Deed
